Coordinate a Production Incident Response
A coordinated incident that restores service safely, keeps stakeholders informed, and produces an evidence-backed follow-up record.
The process
Work it in this order.
- Confirm impact, declare an incident at the appropriate severity, open the incident channel and record, and assign an incident commander.
- Assign operations, communications, and scribe roles; state the current user impact, known facts, and next update time.
- Stabilize the system with the lowest-risk mitigation such as rollback, traffic shift, feature disablement, or capacity increase, preserving evidence where possible.
- Track hypotheses, actions, owners, timestamps, and outcomes; verify recovery with user-facing signals rather than a single internal metric.
- Communicate resolution and monitor for recurrence through an agreed observation window before closing active response.
- Build a blameless timeline, identify contributing conditions, assign bounded follow-up actions with owners, and schedule review.
Bring this
Inputs the cat will ask for.
- Alert or impact report
- Service runbooks and observability access
- Escalation and communication contacts
- Authorized mitigation boundaries
Router + one playbook
Route first. Choose one.
Cat Paw loads one playbook per task rather than merging several. Start with this router; the remaining paths are workflow-specific candidates it can inspect.
- Router:
engineering-pack skills/engineering/addyosmani/observability-and-instrumentation/SKILL.mdskills/engineering/mattpocock/triage/SKILL.md
Guardrails
The boundaries travel with the prompt.
- Prioritize restoring safe service over proving a root cause during active impact.
- Require explicit authorization for destructive or irreversible mitigations.
- Keep customer communications factual; distinguish confirmed facts from hypotheses and avoid blame.
Done means
A checkable finish.
User-facing service is verified stable for the agreed window, stakeholders received closure, the incident timeline is complete, and follow-up actions have owners and due dates.
